What is Modifier XU?

Modifier XU indicates that a procedure or service is distinct because it does not overlap the usual components of the main service. It is one of the Medicare X{EPSU} modifiers that provides a more specific alternative to Modifier 59.

Modifier XU should only be reported when documentation clearly supports that the service is unusual, separate, and not considered a routine component of another procedure under National Correct Coding Initiative (NCCI) guidelines.

When to Use

  • A service is separate from the usual components of another procedure.

  • A distinct diagnostic or therapeutic service is performed.

  • The service does not overlap with the primary procedure.

  • Documentation supports that the service is independently reportable.

  • A more specific modifier than Modifier 59 is appropriate.
