What is Modifier XP?

Modifier XP indicates that a procedure or service is distinct because it was performed by a different practitioner. It is one of the Medicare X{EPSU} modifiers and serves as a more specific alternative to Modifier 59.

Modifier XP should only be reported when separate practitioners independently perform services that would otherwise be bundled under National Correct Coding Initiative (NCCI) guidelines.

When to Use

  • Different practitioners perform separate procedures on the same date.

  • Independent services performed by separate providers require separate reporting.

  • Distinct services qualify for an NCCI exception.

  • Documentation clearly identifies each practitioner.

  • A more specific modifier than Modifier 59 is appropriate.

A general surgeon performs a wound repair while an orthopedic surgeon independently treats an unrelated fracture during the same visit. Modifier XP is appropriate because separate practitioners performed distinct services.
