What is Modifier XE?

Modifier XE indicates that a procedure or service is distinct because it occurred during a separate patient encounter on the same date of service. It is one of the Medicare X{EPSU} modifiers that provides a more specific alternative to Modifier 59.

Modifier XE should only be reported when documentation clearly shows the patient had separate encounters and the services would otherwise be bundled under National Correct Coding Initiative (NCCI) guidelines.

When to Use

  • A patient returns later the same day for a separate medically necessary procedure.

  • Two office visits occur on the same date with distinct services performed.

  • Separate encounters justify reporting procedures that would otherwise be bundled.

  • The medical record clearly documents each encounter independently.

  • A more specific modifier than Modifier 59 is appropriate.

Examples
Example 1

A patient is treated in the morning for a laceration repair. Later that afternoon, the patient returns after reopening the wound and requires another medically necessary repair. Because the second procedure occurred during a separate encounter, Modifier XE is reported.

Example 2

A patient is evaluated in the morning for severe epistaxis and receives nasal packing. The patient returns later that evening with recurrent bleeding requiring additional treatment. Modifier XE is appended because the services occurred during separate encounters.
