What is Modifier XS?
Modifier XS indicates that a procedure or service is distinct because it was performed on a separate organ or anatomical structure. It is one of the Medicare X{EPSU} modifiers that provides a more specific alternative to Modifier 59.
Modifier XS should only be reported when documentation clearly supports that the services were performed on different anatomical sites and would otherwise be bundled under National Correct Coding Initiative (NCCI) guidelines.
When to Use
Procedures are performed on separate organs or anatomical structures.
Services involve different body sites on the same date of service.
A distinct anatomical location justifies separate reporting.
Documentation clearly identifies the separate anatomical structures treated.
A more specific modifier than Modifier 59 is appropriate.
Documentation Requirements
Document the anatomical site for each procedure.
Clearly identify the separate structures treated.
Support medical necessity for each service.
Include complete physician documentation.
Maintain the operative or procedure report.
Examples
Example 1
A physician performs lesion destruction on the patient's right arm and separately removes a lesion from the left leg during the same visit. Modifier XS is appropriate because the procedures were performed on separate anatomical structures.
Example 2
A patient undergoes injections into both the right shoulder and the left knee during the same encounter. Modifier XS may be reported when documentation supports that the services involved separate anatomical sites.
Billing Tips
Use XS only when procedures involve separate anatomical structures.
Report XS instead of Modifier 59 when appropriate.
Clearly document the body site for each procedure.
Review applicable NCCI edits before reporting the modifier.
Verify payer acceptance of Medicare X{EPSU} modifiers.
Common Billing Mistakes
Reporting XS when procedures involve the same anatomical site.
Failing to document the separate structures treated.
Using XS solely to bypass an NCCI edit.
Reporting XS when another X modifier is more appropriate.
Incomplete documentation supporting medical necessity.
