What is Modifier JW?

Modifier JW indicates the amount of a single-dose container drug or biological that was discarded and not administered to the patient. It allows providers to report eligible discarded amounts separately when permitted by Medicare and other payers.

Modifier JW should only be reported for qualifying single-dose containers when the discarded amount meets payer billing requirements.

When to Use

  • A portion of a drug from a single-dose vial is discarded.

  • The discarded amount cannot be administered to another patient.

  • Medicare or the payer allows separate reporting of the discarded amount.

  • The administered and discarded amounts are accurately documented.

Examples

A provider administers part of a single-dose injectable medication, and the unused remainder cannot be safely used for another patient. Modifier JW is appended to report the discarded amount separately.
