What is Modifier GA?

Modifier GA is used to indicate that an Advance Beneficiary Notice of Noncoverage (ABN) was issued to the patient because the provider expects Medicare may deny the service as not medically necessary. The signed ABN informs the patient of their potential financial responsibility before the service is provided.

When Modifier GA is reported correctly, Medicare can process the claim and, if denied, assign financial liability to the beneficiary according to Medicare policy.

When to Use
  • An ABN has been signed before the service is provided.

  • The provider expects Medicare may deny the service due to medical necessity.

  • The service is still performed after the patient acknowledges potential financial responsibility.
