What is Modifier 79?

Modifier 79 is used when the same physician performs a procedure during the postoperative period that is completely unrelated to the original surgery. The modifier tells the payer that the second procedure is independent of the first and should be reimbursed separately.

Unlike Modifier 78, reporting Modifier 79 begins a new global surgical period.

When to Use
  • A new condition requires surgery.

  • The procedure is unrelated to the original operation.

  • The same physician performs both procedures.

  • The second procedure occurs during the original global period.

Documentation Requirements
  • Document the unrelated diagnosis.

  • Support medical necessity.

  • Explain why the procedure is unrelated.

  • Include a complete operative report.

  • Maintain supporting clinical documentation.

Examples
Example 1

A patient recovering from hernia repair fractures a wrist and requires surgical fixation by the same orthopedic surgeon during the global period. Modifier 79 is appropriate.

Example 2

A patient undergoes gallbladder surgery and later requires removal of an unrelated skin lesion during the postoperative period.

Billing Tips
  • Use Modifier 79 only for unrelated procedures.

  • The second procedure begins a new global period.

  • Do not use Modifier 79 for complications of the original surgery.

  • Clearly document the unrelated diagnosis.

  • Follow payer-specific billing requirements.

Common Denial Reasons
  • Documentation does not support an unrelated procedure.

  • Procedure considered part of postoperative care.

  • Insufficient operative documentation.

  • Incorrect modifier selection.

  • Medical necessity not established.
