What is Modifier 78?

Modifier 78 is used when the same physician must return a patient to the operating room during the postoperative period to perform a related procedure because of complications or other unforeseen circumstances following the original surgery.

Unlike Modifier 58, reporting Modifier 78 does not start a new global surgical period. The original global period continues.

When to Use
  • The patient returns to the operating room unexpectedly.

  • A complication develops after surgery.

  • The procedure is related to the original surgery.

  • The same physician performs the return procedure.

Documentation Requirements
  • Document the original procedure.

  • Explain the complication requiring the return to surgery.

  • Support medical necessity.

  • Include a complete operative report.

  • Document that the patient returned to the operating room.

Examples
Example 1

A patient develops postoperative bleeding after abdominal surgery and is taken back to the operating room for hemorrhage control. Modifier 78 is appropriate.

Example 2

A patient experiences wound dehiscence following surgery and requires an unplanned return to the operating room for wound closure.

Billing Tips
  • Use Modifier 78 only for unplanned return procedures.

  • The procedure must be related to the original surgery.

  • The original global period continues.

  • Do not use Modifier 78 for planned staged procedures.

  • Follow payer-specific billing guidelines.

Common Denial Reasons
  • Documentation does not support an unplanned return.

  • Procedure unrelated to the original surgery.

  • Insufficient operative documentation.

  • Incorrect modifier selection.

  • Medical necessity not documented.
